We have a full time Family Medicine Opportunity available at:

  • Family Health Center
    • Our Family Medicine providers at the Family Health Center work alongside our Family Practice Physician Residents and Faculty.  Our providers diagnose and treat long- and short-term illness. They also provide preventive care, including routine checkups, health-risk assessments, immunization and screening tests, and personalized counseling on maintaining a healthy lifestyle.  For patients with chronic illness, our providers also manage and often coordinate care provided by other subspecialists.

      This position is located at the Northway Clinic in St. Cloud.  In this role, you will work clinic hours without any required call.  

In the primary care setting, our providers diagnose and treat short-term illness.  They provide preventive care, including routine checkups, health-risk assessments, immunization and screening tests, and personalized counseling on maintaining a healthy lifestyle.  For patients with chronic illness, our providers also manage and often coordinate care provided by other subspecialists.

NEW GRAD TRANSITION INTO PRACTICE PROGRAM 

In this postion you will participate in our Transition Into Practice Program (formerly our Family Medicine Residency Program). All new graduate APPs who are hired into a CentraCare Family Medicine position will go through this program.  We are defining new graduate as having less than 12 months of experience as a Nurse Practitioner, Clinical Nurse Specialist, or Physician Assistant.  This thorough transition into practice program will serve to instill the knowledge, skills, and experience new graduates need to be a confident and competent primary care provider. 

You will begin orientation to your new position at your home Family Medicine clinic, then start specialty rotations and didactic education with the APP Residency program at the next cohort start date.

REQUIREMENTS

To be eligible for this position you must meet one of the following requirements;

  • Certified as a Nurse Practitioner or Clinical Nurse Specialist
  • Current Minnesota Physician Assistant Licensure
  • Be eligible for certification (NP/CNP) or licensure (PA) within the next 12 months

In addition, all applicants must have a current American Heart Association BLS Healthcare Provider card and Drug Enforcement Agency (DEA) registration number prior to receiving clinical privileges.
